Are this meshes genetic or do you have to change your sims to wear the clothes that have 'enlarged' features? If so what should i download, only wish for m sims to have a bigger 'top' half rather than anything else.

The clothes themselves can be worn by anyone as long as you have the meshes.... when you remove the clothes they will have their genetic bodyshape. If you create a sim with Warlokk's or compatible skin, then it DOES become genetic and can potentially be passed down to their offspring. I hope I answered clearly enough.

Right... basically the answer is, "Both".  Meshes themselves are not genetic, the game doesn't work that way... the skintones that reference them though, are genetic.  Clothes must be assigned manually to your sim, the skintone meshes only affect your sim when she is naked.

Check out the Bodyshape How-To tutorial linked in my sig, it explains how the whole thing works.  It's not as easy as just assigning a size to a sim, and all the clothes are magically that size... that would require a different game than what Maxis has given us, unfortunately.  Maybe Sims 3.  For now, it takes a little effort on the player's part, but I think it's worth it.
