Poseboxes (mostly extracted ingame poses but also some custom boxes), find the right poses that look as natural as possible (and are more or less similar to how I want the picture to turn out) - which sometimes takes a while, plus (by now) a few years of experience.

(nah... did the easy thing. One pic eyes open, one pic eyes shut. Then the magic of Photoshop did the rest, with some help from me ;D)
