I've actually had that happen twice. I don't know if it's a problem with the custom furniture's mesh, or the hair mehs or what. It's creepy because when you zoom out it's just this blob of white on the toddler's chin.

Wow, twice? It most likely the way the chin is shaped. But it probably could be a mesh mess up.

The vast majority of toddlers faces do that when in a cot and miserable. It's just a failed MAXIS animation.
