My orphan scan came up with over 700 files, so I created a new folder called "orphans", moved them all to that folder and then moved that folder to the desktop for now, while I run the game and see if anything I need comes up missing.

If you put them in the SavedSims folder, they will show up in Create A Family as premade sims (above the line to enter their name, there's an option to randomize and then the one you want: choose existing sim). Their appearance would be the same, but you would have to name them again and choose their aspirations, family links, etc...

Here's a few things to try:
Delete the groups.cach file in My Documents/EAGames/TheSims 2 and if you use Bodyshop, also delete the cigen.package. These files get corrupted easily when you make alot of changes to your Downloads folder. They'll regenerate on their own when you run the game/BodyShop.
If that doesn't do the trick then go back to MyDocuments/EAGames/TheSims2and delete the Thumbnails folder. This will be recreated when you run the game again also. The game will slow down especially in Create a Family or build mode while the game regenerates the thumbnails, and then go back to normal again.

cause i have no expansions now, even in some tutorial pics i see things that are not in my game
There's the problem, you're running just the base game with no expansion packs. The items that you downloaded that aren't showing up are probably recolors of items introduced with the expansion packs. Alot of creators don't list which EP's are required for their items to show up, each EP adds new content and it's hard to keep track of sometimes.

Hmm this one could get complicated. My first thought is conflicting mods, but I'm guessing you've run the Hack Detection Utility already. My next thought is since you've had CAS/Body Shop problems too, it could be a corrupt/incompatable file specifically related to genetics like hair, eyes and/or skintone. Does this happen to all your pregnant sims or just a specific one? If it's just a specific one that it happens to, you could try moving them onto a different lot. It may be a hack causing the problem. Run the Sims 2 Hack Conflict Detection Utility: http://www.simwardrobe.com/ You can also try deleting the groups.cache file from mydocuments/eagames/thesims2. If all else fails, drag your Downloads folder to your desktop then run the game. If it runs fine without the Downloads folder, then you know you've got a bad file or files. If that's the case, you can start replacing your downloads a few at a time until you find the culprit. Or you could use the Clean Installer, have it scan your downloads, then sort by date and start disabling downloads individually starting with the most recent.
