It's all depending on the mesh used to make the clothes and, wereas you can let your sims wear anything while pregnant, it's a hit or miss as to whether the mesh will show the pregnancy.
If you look for maternity wear here or on MTS2 (http://www.modthesims2.com/) you should find some clothes that will work. All About Style (http://www.all-about-style.com/themes_maternity.html) has some too.
