You would have to unlock the NPC outfits and then create a sim in CAS that looked like you wanted and then use Sim Surgery to replace them. That's the way I do it. It's a pain but at least they. Though my maids keep reverting back to their old hair and I have to make them take a shower or go swimming and they change bac to the new hair.
