You can get twins of mixed gender but you need to know a sim who is fairly high up in the medical field.  I am not taking credit for discovering this myself, by the way, but I have tested it and it works.  If you are pretty sure your sim will have twins because she has the fertility treatment, get your doctor friend to 'examine' your sim.  The doctor will tell your sim if she is having a girl or boy and then you must make your sim either eat three servings of watermelon or apples - depending on what you want.
