I have a sim using Enayla's Pixie Emerald skintone and there are clothes showing as the skintone where there should just be skin. Everything was fine in CAS and I honestly don't know how this happened. Has this happened to anyone else before and do they know how this happened, and if it can be fixed? I've already tried turning on Boolprop, forcing an error and resetting. No luck.

Have you tried deleting the groups and accessories file's just inside your Sims 2 folder?

You might want to also delete all the files inside the Thumbnails folder also.

The game makes new ones when you start it up.

Deleted groups and accessories cache files, and cleared my thumbnails folder. No change

You may have an outdated hack or if you downloaded a house recently, you may have picked something up without realizing it.

Have you scanned your downloads folder recently?

The weird thing is that the sim has a son with the same skintone and it isn't having that issue.
I did get a house a week or so ago, maybe I did end up picking something up. WHat's a good download scanner?

I had a similar issue with some hair coloring the scalp of all males.
Turned out there was a wrong setting inside SimPE on a hair I had tried to bin, that made some of the hair textures think they were default replacements for the scalp color.

Does the same thing happen if other male adult sims has that skintone? If so, then there is a chance some clothing you downloaded might have a similar problem to the hair...
