Do you have to download all the different clothes to get the different body shapes or can you just download a full body that you can put different clothes on?

Only if you want them to retain the shape when they're dressed... The clothes themselves will not morph to show the mesh you have your skintone linked to, only by installing & choosing the appropriate sized clothes will your Sim stay consistantly the same shape whether in a raincoat or in the shower...
